Please review the following before your scheduled Zoom assessment with our nurse:

• Ensure you are present for the full assessment.
• Check your camera to make sure it is clear and working properly.

Once the assessment is complete:

• Admission documents will be sent to your email for signing and review.
• We will finalize the service agreement with your hourly rate.
• A two-week retainer will be required before starting services.
